Our Direct Primary Care follows a membership based model. If you are interested in a Direct Primary Care membership, please give us a call to discuss which provider is available and your specific healthcare needs.
Membership fees are per month.

$229: Individual
$270: Couple or Single Parent +1 Child
$329: Family of 3 or More
Plus one time registration fee.
All Other Providers
$149: Individual
$229: Couple or Single Parent +1 Child
$249: Family of 3 or More
Plus one time registration fee.
One time registration fee: The registration fee is based upon the price of your monthly payment.
For Veterans, Active Military, Police, and Fire Workers, Teachers and Senior Citizens
