Direct Primary Care (DPC) is an innovative alternative payment model for primary care being embraced by patients, physicians, employers, payers and policymakers across the United States. All visits, emails and phone calls are included with a Direct Primary Care Membership.

Functional Medicine Memberships are a little different. Refer to the Functional Medicine section of the Membership Contract.

While lab tests are not covered, we work hard to keep them affordable. Patients who choose to pay cash receive significantly discounted rates (up to 90% off standard lab pricing). Labs may also be submitted to insurance upon request.

Since we have taken insurance requirements out of the equation, we can do visits by phone, Skype, text (if quick visit), and email if needed. However, please trust us if we say we need to see you in person.

Direct Primary Care memberships are a required six (6) month minimum contract. Memberships at Benton Integrative Medicine are designed and are intended to be continuous. You are only eligible for termination after the initial six-month period has passed. Please be sure to submit the Benton Integrative Medicine DPC Cancellation Form at least 15 days before your next membership invoice is drafted (draft date) or your cancellation will not be considered valid. Benton Integrative Medicine does not offer prorated refunds for partial months.

Early Termination: If you terminate your membership before the required 6 months you will have to wait for an open enrollment period if you want to rejoin in the future and you will be subject to pay an additional month after your next draft date."

Yes you can! You can use your insurance for your labs, X-rays, visits to specialists, ER, and hospital stays. However, we will be paid by the monthly membership fee since we want to have more freedom to be better providers and spend more quality time with you than is permitted under the insurance model. Isn't that totally better than that last 10 minute visit you had after waiting 45 minutes to see the doctor in the first place? You will have less wait time and more time with your provider during your visit.

We can still see you, however, as per the Affordable Care Act, everyone needs to purchase health insurance, especially to cover hospital, emergency room and specialist costs. Many insurance plans offer high deductible plans for outpatient services and this clinic would be ideal for those situations since the annual fee is much lower than you would pay anywhere else. Also there are cost sharing companies such as Liberty Direct that offer low monthly payments for and provides subsidies to help cover the monthly fees and even functional medicine supplements sold at the clinic. Samaritan's Purse also offers low monthly payments to make it easier to pay for both.

Advanced Care Card

We accept Advanced Care Card for Functional Medicine/Wellness services but NOT for Direct Primary Care. As a perk, they offer 15 months no interest on 1st payment-no matter how big or small (dependent upon your limit).

For those of you with insurance, here is how a Direct Primary Care (DPC) practice works best. If you have an option on your insurance for a high deductible in regards to outpatient services, then choose that option. Please make sure you are well covered for hospitalization. However, the cost of going to a DPC practice for an entire year is often MUCH less than you will pay to have good outpatient insurance "coverage". Also, for those of you whose employers will pay for a yearly physical but the rest is on you in regards to outpatient visits, a DPC practice may work well. If you can choose a plan with an HSA, many have used those to pay the monthly fees which works great!
